1. Who we are

My Fit-RX LLC, a Nevada limited liability company, doing business as MyFit-Rx ("MyFit-Rx," "we," "us"), is an online wellness brand. MyFit-Rx connects patients with independent, licensed healthcare providers and licensed pharmacies through a third-party telehealth partner.

MyFit-Rx does not practice medicine, does not prescribe medications, and does not dispense or ship medications. All medical evaluation, prescribing, payment processing, and fulfillment are performed by the telehealth partner's affiliated providers and pharmacies, including [MEDICAL GROUP NAME] and [PHARMACY NAME].

2. Acceptance of these terms

By accessing or using myfit-rx.com, you agree to these Terms of Service and our Privacy Policy. If you do not agree, do not use the site. When you proceed to evaluation or checkout, you do so on the telehealth partner's platform, and the partner's own terms and consents also apply.

3. Eligibility

You must be at least 18 years old and a resident of a US state where the partner's services are available. Availability varies by state and by program. By using the site you represent that you meet these requirements.

4. No doctor-patient relationship with MyFit-Rx

Using this website does not create a doctor-patient relationship with MyFit-Rx. Any clinical relationship is formed solely between you and the independent licensed provider who evaluates you on the partner platform.

5. Provider discretion

All treatment decisions are made by independent licensed providers. A provider may decline to prescribe or recommend treatment for any clinical reason. Completing an evaluation does not guarantee a prescription.

6. Not for emergencies

MyFit-Rx and the partner platform are not for medical emergencies. If you are experiencing an emergency, call 911 or go to the nearest emergency room.

7. Intellectual property

All content on this site — including text, graphics, logos, and design — is the property of My Fit-RX LLC or its licensors and is protected by intellectual property laws. You may not copy, reproduce, or distribute site content without written permission.

8. Acceptable use

You agree not to misuse the site, including by attempting unauthorized access, interfering with the site's operation, submitting false or misleading information, or using the site for any unlawful purpose.

9. Disclaimers

10. Limitation of liability

Clinical services are provided solely by the independent providers and pharmacies.

11. Indemnification

12. Dispute resolution and arbitration

Any dispute arising from these terms or your use of the site will be resolved by binding individual arbitration rather than in court, and you waive the right to a jury trial and to participate in a class action.
