1. MyFit-Rx is not a healthcare provider
My Fit-RX LLC, a Nevada limited liability company, doing business as MyFit-Rx ("MyFit-Rx," "we," "us"), is an online wellness brand that connects patients with independent, licensed healthcare providers and licensed pharmacies through a third-party telehealth partner. Because MyFit-Rx does not provide medical care, prescribe, or dispense, MyFit-Rx is not a HIPAA covered entity.
2. Who holds your health information
Your protected health information — including intake answers, medical history, and treatment records — is collected and maintained by the treating providers and the dispensing pharmacy, including [MEDICAL GROUP NAME] and [PHARMACY NAME], on the telehealth partner's secure platform.
MyFit-Rx does not receive or store your health information. The only information MyFit-Rx collects is described in our Privacy Policy.
3. The Notice of Privacy Practices
The providers and pharmacy that treat you are HIPAA covered entities and maintain their own Notice of Privacy Practices describing how your health information may be used and disclosed and the rights you have over it. Their Notice of Privacy Practices is available in the patient portal after you complete intake.
4. Your rights
Under HIPAA, you generally have the right to access your records, request corrections, request an accounting of disclosures, and request restrictions on certain uses. These rights are exercised with the providers and pharmacy that hold your records, through the patient portal.
5. Questions
For questions about your health records, contact your provider through the patient portal. For questions about this page or MyFit-Rx's own data practices, email admin@myfit-rx.com.
